Mr. Garrison is getting a sex change. The boys have to help Mr. Garrison get his testicles out of Kyle's knees! Mr. Garrison decides he wants to become a woman, as he's more comfortable as dressing in drag as one, so he loses his testicles and scrotum. Kyle becomes a tall black kid in order to qualify as a member of the Colorado All-State basketball team by means of a negroplasty (his kneecaps are replaced by Mr. Garrison's testicles), and Kyle's dad becomes a dolphin in order to look how he feels (he gets Mr. Garrison's scrotum as a dorsal fin). |

- The names of Dr. Biber and the Trinidad Medical Institute are almost surely a reference to Dr. Stanley Biber, a now-deceased prominent sex reassignment surgeon at the Mount San Rafael Hospital in Trinidad, Colorado. His surname means "Beaver" in German, which gives an added meaning to his work in sex-change operations.
- Live action highlights can be seen during Mr. Garrison's sex change operation. These highlights are actually that of an animal being neutered.
- When Kyle is supposed to enter the basketball game, the stadium host says "Number 8, Kyle B." Actually, Kyle's jersey says "4".
- After the sex change, Garrison rechristens herself Mrs. Garrison, despite the fact that "Mrs." is a married woman's title, and she is not married.
- As soon as Kyle says "All right, Dad!" his uniform changes from red shorts and white jersey with red trim to all white with black trim. Two stars and the words "All State Colorado" appear on the jersey and the number 4 changes from red to black. Then he leaves for Denver with his dad. When he arrives at the convention center, he's wearing the red shorts and white jersey with red trim again.
- When agreeing to perform surgery on Kyle, Dr. Biber mentions the procedure "caucasiaplasty" as the reverse of a "negroplasty". He claims that pop music artist Michael Jackson has had this surgery performed on him, in reference to the fact that the color of Michael Jackson's skin has gradually become lighter through his career.
- On the arena wall: An upside-down M, with the words "Gotta love it!" This is a spoof of McDonald's and its "I'm Loving It" campaign.
- According to the DVD commentary on this episode, Trey Parker said he almost "pulled a Dave Chappelle", because he was disappointed with this episode and after having had such a stressful year in 2004 while making 14 episodes of South Park and working on Team America: World Police, wanted to leave South Park. He was apparently convinced to stay when they came up with the idea that Gerald Broflovski was half lawyer and half dolphin making him a "lawfin".
- After becoming a woman, Mrs. Garrison appears in a video from the Girls Gone Wild pornography franchise.
